Our work with our hospitals and community-based organizations allows GDAHA to serve in a unique position as a partner, facilitator, and collaborator to address the most pressing health needs of our region. These challenges, identified through the Community Health Needs Assessment, are among the most difficult to address and require all partners working together toward a shared vision.

Decide to be Heard works to ensure all healthy adults have access to trained facilitators to help them complete advance directives and have critical conversations with their healthcare agent and loved ones.
Healthy Beginnings at Home evaluates how help with housing impacts the health of moms, babies, and families who are in difficult housing situations.
A broad system of free services and supports for pregnant women, caregivers with new babies, and families with young children to the age of three. These evidence-based programs provide education, support, and advocacy.
The Dayton Regional Pathways HUB is a free program to help connect people to medical and social services they and their loved ones need most. Participants partner with a community health worker to empower them to make decisions regarding their health and well-being.
